Product Developed by the Contingency Management Task Force (CM Task Force) The Addiction Technology Transfer Center Network Coordinating Office (ATTC NCO) convened the Contingency Management Task Force (CM Task Force) in April 2023 at the request of the Substance Abuse and Mental Health Services Administration (SAMHSA). The purpose of this Contingency Management (CM) Task Force was to define the key components of CM that will facilitate consistent evidence-based training and implementation, and to scale CM in a manner that is meaningful, permissible, and scientific. The insights gathered will help SAMHSA’s [Training and Technical Assistance (TTA) Centers] to offer technical guidance, resources, partnerships, and infrastructure to support the expansion of CM to stakeholders in the field of addiction and recovery. This initial meeting is crucial in developing a shared vocabulary and an evidence based approach to CM education and training platforms, with the goal of incorporating metrics to measure effectiveness. Key information and components for facilitating CM TTA found in this report, include: 1. Key areas of considerations relating to implementation guidance (Section 2) 2. Recommendations pertaining to the development and rollout of implementation guidance (Section 3) 3. Proposed chapter titles and topics to be used in the possible development of a Contingency Management Implementation Guidance Document (Section 4) 4. Publicly available resources (Section 5) 5.   SUD Keys to Education is a product for educators and clinical supervisors developed in 2022 by the Mountain Plains and Pacific Southwest Addiction Technology Transfer Centers (MPATTC and PSATTC). This product was developed to help community college/university faculty, as well as clinical supervisors and recovery support staff to have access to brief, science-based content with the goal of providing materials that can be easily infused into existing substance use disorder and related courses (e.g., social work, nursing, criminal justice, foundation of addiction courses, ethics, counseling courses, etc.) and for clinical and recovery staff use in in-service meetings. Individuals can select the specific content to infuse into existing curricula/materials depending on specific needs of their learners. Each slide in the slide decks contain notes to provide guidance on the topics along with references and handouts where appropriate. The purpose of this introductory training is to provide HIV clinicians (including, but not limited to physicians, dentists, nurses, and other allied medical staff, therapists and social workers, and counselors, specialists, and case managers) with an overview of the challenges and strategies for change in working with individuals with HIV/AIDS and a diagnosis of an opioid use disorder. Given the potentially broad array of health disciplines, this presentation uses the terms “patient” and “client” interchangeably to refer to the individuals a training participant may be working with in their organization. The duration of the training is approximately 180 minutes (3 hours), depending on whether the trainer chooses to present all of the slides, or a selection of slides, and whether the trainer chooses to present and discuss both case studies.   The training package includes a 90-minute PowerPoint presentation (abridged curriculum), a 3-hour PowerPoint presentation (full curriculum), a fully articulated trainer guide for the full curriculum, and a two-page fact sheet. All of the training package components are 508 compliant.

Contingency management provides monetary incentives (such as gift cards) for meeting treatment goals, including stimulant non-use as measured by negative urine drug tests (UDTs), rewarding individuals for changing their behaviors. Substance use offers a powerful, immediate reinforcement. Contingency management confronts this challenge by offering immediate financial awards that can help activate the brain’s reward system, thus encouraging the replacement of stimulants with the incentive to achieve a dopamine release. This course is a two-hour self-paced online training designed to provide a broad overview of contingency management for treating individuals with a stimulant use disorder (StimUD). The course includes a brief Introduction and seven (7) Modules. You can complete one Module at a time as your schedule permits. Completion of this course is required for individuals who are serving as a CM Coordinator, Back-Up CM Coordinator, or CM Supervisor within their agency.    Learning Objectives:  At the end of the course, participants will be able to:  1.    Recall at least three (3) patterns and trends regarding the scope of stimulant use in California and beyond. 2.    Identify at least three (3) acute and three (3) chronic effects of psychostimulant drugs on the brain. 3.    Specify at least three (3) key principles of operant conditioning. 4.    Formulate a list of the four (4) essential elements of an evidence-based, protocol-driven Recovery Incentives Program to address stimulant use.

A common approach in learning any complex skill is to work with experts assisting in the development of competencies. Coaches observe practice in vivo and provide prompt feedback in a safe environment to guide practitioners to enhance fidelity to the model and retain skill levels in Motivational Interviewing (Miller & Moyers, 2021). The Mountain Plains, in collaboration with Motivational Interviewing (MI) subject matter experts, developed this opportunity to enhance practitioners’ skills and confidence in providing motivational strategies through individualized coaching. Coaching in specific MI approaches will be available to training participants following completion of a foundational MI course and a 9-week MI-Into Practice Enhanced Professional Learning Series. Basic MI concepts and skills to resolve ambivalence and conduct intentional conversations about change will be featured. This series of coaching is intended to help build confidence and practical MI knowledge and skills to support and address the behavioral factors impacting client change. Participants that have met the pre-requisites noted above will be invited to participate in 3 coaching sessions and will complete a pre- assessment to determine individualized coaching topics. Most sessions are individual. However, participants can be grouped in pairs or trios for effective interaction if preferred.    *Pre-requisites required

This manual was developed as part of a PhD by the South Africa HIV ATTC Co-Director, Dr. Goodman Sibeko. Initial piloting took place at two non-profit organizations after ethical approval was obtained from the Human Research Ethics Committee (913/205) of the University of Cape Town.The manual now serves as the basis of the training approach used by the South Africa HIV ATTC to help organisations improve mental health care as a means of reducing HIV risk and improving HIV treatment outcomes.
